Keeping Bristol Safe Partnership

The Keeping Bristol Safe Partnership (KBSP) has today (03rd September 2025) published a Safeguarding Adults Review (SAR) which was commissioned following the death of Adam (pseudonym) in January 2024.

The Partnership wishes to extend their sympathy to anyone affected by Adam’s death and thank those who have contributed to this review.

Adam Overview Report

It is important that lessons are learned from Safeguarding Adult Reviews. A learning briefing has been created for professionals to use in individual learning, team meeting briefings or individual supervision to ensure that the findings are disseminated widely. Some of the key themes covered in this briefing are working with individuals who use substances, travellers in our community, literacy issues, self-neglect concerns, and examples of good practice.
