The Keeping Bristol Safe Partnership (KBSP) has today (22nd October 2025) published a Safeguarding Adults Review (SAR) which was commissioned as a thematic review following the deaths of George and Peter (not their real names) in 2024.

The Partnership wishes to extend their sympathy to anyone affected by George and Peter’s death and thank those who have contributed to this review.

It is important that lessons are learned from Safeguarding Adult Reviews. A learning briefing has been created for professionals to use in individual learning, team meeting briefings or individual supervisions to ensure that the findings are disseminated widely. Some of the key themes covered in this briefing are working with individuals who are alcohol dependent, self-neglect, mental capacity, barriers to engagement, the use of the section 42 framework, deteriorating health conditions, exploitation and fire risk, as well as highlighting examples of good practice.
