The Keeping Bristol Safe Partnership (KBSP) has today (28th January 2026) published a Safeguarding Adults Review (SAR) which was commissioned following the death of Natalie (pseudonym).

The Partnership wishes to extend their sympathy to anyone affected by Natalie’s death and thank those who have contributed to this review.

Natalie Executive Summary

It is important that lessons are learned from Safeguarding Adult Reviews. A learning briefing has been created for professionals to use in individual learning, team meeting briefings or individual supervisions to ensure that the findings are disseminated widely. The SAR focused on several overarching safeguarding themes: understanding and responding to Natalie’s story, information sharing and multi‑agency working, and the multi‑agency management of risk. These themes intersect with specific issues including trauma, care experience, domestic abuse and sexual violence, mental health needs, the heightened risk of suicide following child removal, and drug and alcohol use.
