The Keeping Bristol Safe Partnership (KBSP) has today (27th August 2025) published a Safeguarding Adults Review (SAR) which was commissioned following the death of Richard in February 2024.
The Partnership wishes to extend their sympathy to anyone affected by Richard’s death and thank those who have contributed to this review.

It is important that lessons are learned from Safeguarding Adult Reviews. A learning briefing has been created for professionals to use in individual learning, team meeting briefings or individual supervision to ensure that the findings are disseminated widely. Some of the key themes covered in this briefing are mental health, discharge planning arrangements, medical interactions with physical health concerns, and examples of good practice.
